Not a tooling problem. A question that was cheap to ask at the moment it came up, and expensive by the time it surfaced. Each product above answers one shape of it, and they are the same shape.
“The dashboard is broken.” Which dashboard, broken how, for whom, since when. The reporter knew. The developer guesses, and the guess costs a sprint.
It was made in a thread. Nobody priced it, nobody signed it, and the teams it changed found out in the release notes. Six weeks later it gets re-litigated from memory.
Change what people are required to do and you've created work nobody scheduled. It shows up as an audit finding, or as the person who “never got told.”
